Village Dental Center Discount Program

We have noticed lately in our practice that many local families are feeling burdened by the cost of dental insurance or dental check-ups for their families.  This got us thinking that there has to be a way for us to offer patients with NO dental insurance, a way to save money while still getting excellent dental care.  For this reason, we have created the Village Dental Center Savings Plan.  With this plan there are no deductibles, no yearly maximums, and no waiting periods to begin treatment. 

We now offer two plans.

Traditional Dental Savings Plan

Includes:

2 preventative visits per membership year.

Each visit includes prophylaxis (cleaning), exam, fluoride treatment (children 16 and younger), Velscope oral cancer screening, and 4 check-up x-rays per year. Does include a full mouth x-rays as needed, with the determination being made by the professional staff at Village Dental Center.

·         15% Discount on ALL in office Dental procedures and products up to $5000 per year. (Payment due in full at time of service)

ANNUAL MEMBERSHIP COST IN TRADITIONAL DENTAL SAVINGS PLAN:

Single/individual $459 & Couple $970

Periodontal Maintenance Dental Savings Plan

Includes:

3 maintenance visits per year.

Each visit includes periodontal maintenance (periodontal cleaning), doctor’s exam, Velscope oral cancer screening and 4 check-up x rays per year. Does include a full mouth set of x-rays as needed, when determined appropriate by the professional staff of Village Dental Center.

15% Discount on ALL in office dental procedures and products up to $5000 per year. (Payment due in full at time of service)

ANNUAL MEMBERSHIP COST IN PERIODONTAL MAINTENANCE DENTAL SAVINGS PLAN - ONLY $899

VILLAGE DENTAL CENTER SAVINGS PLANS GUIDELINES:

·         Family members eligible for savings plans include spouse and dependent children under the age of 26.

·         All savings plan membership fees are due and payable in full at the time of registration.

·         Savings plan membership fees are NON-REFUNDABLE.

·         All charges are due day of service.

·         Savings plan duration is for one full year, starting on the registration date.

·         Coverage expires at the end of the membership year.

DENTAL SAVINGS PLAN EXCLUSIONS AND LIMITATIONS

·         Members cannot use any other dental insurance or discount plan with this plan.

·         Will not cover any portion of a specialist referral.

·         Will not apply to costs of dental care for injuries covered under workman’s compensation.

·         Will not apply to dental care for which, in the sole opinion of the treating dentist or doctor, lies outside the realm of their capability.

·         Will not apply to hospitalization or hospital charges of any kind.

·         Will not apply to costs of dental care that is covered under automobile medical insurance.

·         Covered benefits of the savings plan are available only at Village Dental Center, Lake Zurich, Illinois
